Data, Performance, Quality and Continuous Improvement

At maturity, OHT’s will have the ability to drive performance improvements through clinical and data standardization, advanced analytics and strong information management practices to enable population health management, quality improvement and outcomes measurement.

OHTs will be measured, report on and improve performance across a standardized framework, linked to the Quadruple Aim (improved health outcomes, patient and provider experience, and value).

The Data, Performance, Quality and Continuous Improvement Working Group was formed to enable this work. We are proud to share the following resources with you:

•    Quality Framework
•    Rapid Cycle Learning Approach
•    Collaborative Quality Improvement Plan Approach
•    Latest Performance Measure Report

Watch for our approved 2022/23 Collaborative Quality Improvement Plan to come later this year.
