Séance d'engagement en soins primaires KW4 OHT avec John McKinley

Objective: Provider Care Networks (PCNs) have emerged as a tool for primary care providers in Ontario to organize themselves within Ontario Health Teams (OHTs). The KW4 OHT is currently exploring the development of a PCN in KW4, and is hosting a local town hall for relevant primary care professionals.

Facilitation: A consultant team who has been leading the project on behalf of KW4 will be facilitating the town hall. Attendees will participate in plenary group sessions and breakout groups.
