The Centre for Family Medicine Family Health Team

The Centre for Family Medicine, Family Health Team's vision is to create a healthier community through the provision and transformation of comprehensive patient-centred primary care. They have a rostered population of over 26,000 patients in the KW4 area with a primary care physician as part of the Centre for Family Medicine FHO. The FHT services all of these patients with a team-based approach to care, as well as offers other programs to our community within a primary care lens.
