
Priorities
In spring 2019, community consultations were held with health, municipal and social service agencies to talk about what changes were needed in the healthcare system to best fit the community's needs. While the KW4 OHT will continue to provide full care to the over 400,000 residents in our catchment area, the group decided to focus on those who are socially vulnerable, have had difficulty accessing services, and have faced barriers with continued care. They selected three first year target groups:

Homeless & Precariously Housed

Frail Elderly
-
Newcomers
As well, they acknowledged the shared overarching lenses of mental health and addictions and urban/rural service planning contexts for all priority groups.

The Declaration serves as a compass for KW4 OHT members in the delivery services.
