Updated guidance supporting high-quality transitions from hospital to home

 

The Transitions Between Hospital and Home: Care for People of All Ages quality standard has been updated and includes 10 statements defining what high‑quality care looks like when individuals transition from hospital to home.

 

The standard emphasizes clear and timely information‑sharing, effective communication between patients, caregivers, and care teams, comprehensive assessment, medication review, coordinated follow‑up care, and education and support for patients and care partners.
