Health Care Provider Advisory
Key Changes and Highlights
- Reporting of positive TB Skin Tests (TSTs), IGRAs, and referrals to TB Clinic streamlined into a single form.
- New Tuberculosis Preventive Treatment (TPT) Order Form available for community health care providers prescribing latent TB infection (LTBI) treatment for uncomplicated cases.
- Submission must include a fully completed Tuberculosis Reporting and Referral Form.
Responsibilities of Ordering Health Care Providers
- Order and assess all baseline and treatment‑monitoring bloodwork.
- Address issues or concerns arising during treatment.
Public Health Responsibilities
- Dispense all TB medications at no cost.
- Provide treatment‑adherence support.
- Assist with bloodwork reminders.
- Notify the ordering health care provider of issues as they arise.
Health Care Provider Action Required
- Dispose of all copies of the obsolete Report of Positive TB Skin Test Form and Health Care Provider Treatment and Referral Form.
- Use the new Tuberculosis Reporting and Referral Form to:
- Report all positive TSTs and IGRAs with a recent chest X‑ray report.
- Refer patients to TB Clinic based on clinical assessment.
- Use the Tuberculosis Preventive Treatment in Adults – Order Sheet for Community Practitioners to prescribe LTBI medication for uncomplicated cases.
- Complete all sections of required forms before submission.
- Request copies of updated forms from the Public Health website: https://rebrand.ly/RequestAForm
- Refer to the Canadian TB Standards, 8th Edition (2022) for guidance on LTBI diagnosis and treatment.
- Submit completed forms by fax to 519‑883‑2248.
